Community Health Assessment Survey
Marion County community members and providers of health-related services are invited to share their views on health and healthcare by taking a 10-minute survey. Survey results, along with local health data, will be used to develop a Community Health Needs Assessment. This report will identify the most pressing health issues in the county and serve as a tool for community leaders to take actions towards a healthier community. Your voice matters!
The community survey for residents at least 18 years old is at https://tinyurl.com/MarionHealthSurvey.
Community-based organizations, social service providers and healthcare professionals are encouraged to take the provider survey at https://tinyurl.com/MarionProviderSurvey. Both surveys will be available from May 3 to June 10, 2022.
This survey is funded by the Florida Department of Health in Marion County and AdventHealth. WellFlorida Council, the region’s health council and consultant for health causes, is conducting the surveys.
